Amazon Plan of Action template
A Plan of Action answers three questions about the issue Amazon named: why it happened, what you have done about it, and what stops it from happening again. Use the template below, or paste your notice and get one written from your own facts and documents.

The three parts Amazon's reviewers look for
Root cause
The specific reason the issue happened in your business: an unverifiable supplier, a stock-control gap, a missing quality check. Not "we will be more careful". One cause per paragraph when there are several.
Actions taken
What you have already done to resolve the issue, with dates: units removed, listings corrected, refunds issued, suppliers changed.
Preventive measures
What you changed so it does not happen again, concrete enough to verify: the process, who owns it, how often it runs, which tool it uses.
The template
Replace every [ADD: …] with your own facts. Leave out anything that is not true for your case rather than filling it with general statements.
Subject: Plan of Action for [ADD: the issue named in the notice, e.g. "inauthentic complaint on ASIN B0XXXXXXXX"] Hello Seller Performance team, Thank you for the opportunity to address this issue. Below is our Plan of Action for [ADD: the notice and its date]. 1. Root cause [ADD: the specific cause of the issue in our business, one paragraph per cause. For example: "We purchased ASIN B0XXXXXXXX from a distributor (Supplier name) that could not show authorization from the brand."] 2. Actions we have taken - [ADD: action, with the date it was completed. For example: "On [date] we removed the remaining 120 units from sale and requested a removal order."] - [ADD: action and date] 3. Preventive measures - [ADD: the process we changed, who owns it and how often it runs. For example: "Since [date], every new supplier must provide a brand authorization letter before the first order; our purchasing lead checks it."] - [ADD: measure, owner, frequency] 4. Supporting documents - [ADD: document] — [ADD: what it proves, e.g. "invoice INV-123 from Supplier, dated [date], covering 500 units of ASIN B0XXXXXXXX"] - [ADD: document] — [ADD: what it proves] We have addressed every issue listed in the notice and we are committed to keeping our account in good standing. Thank you, [ADD: business name as registered in Seller Central]
How to write it
- Address every issue and every ASIN the notice lists.
- Be specific and factual: names, dates, quantities, document numbers.
- Keep it short. Bullet points are easier for a reviewer to check than long paragraphs.
- Attach evidence for what you claim, and say what each document proves.
- Use documents within the window the notice asks for, often the 365 days before the notice.
- Write in English, without emotional language, blame, or questions unrelated to the case.
Mistakes that get a Plan of Action rejected
- A generic root cause that could fit any seller.
- Actions and measures without dates or owners.
- Pro forma invoices, quotes, or invoices outside the date window as evidence.
- Blaming Amazon, buyers or the rights owner.
- Resubmitting the same Plan of Action, or resubmitting before Amazon has reviewed the last one.

Example: proforma-invoice.png
Will be rejected · A completed, paid transaction: not a quote, pro forma invoice or order confirmation.
Amazon's wording: “Sales quotes, pro forma invoices” · help page
Passes · Issued within the 365 days before the notice.
Amazon's wording: “A clearly displayed issue date within the past 365 days before the receipt of the performance notification” · help page
Passes · The supplier's name, phone number, address and website.
Amazon's wording: “Contact information of your supplier, including a name, phone number, address, and website” · help page
An example with real rules, not a customer's case.
Questions
What is an Amazon Plan of Action?
A document a seller submits to Amazon after an account deactivation or a listing removal. It explains the root cause of the issue, the actions already taken, and the measures that prevent it from happening again.
How long should a Plan of Action be?
As long as it takes to cover every issue in the notice, and no longer. Most fit on one page with bullet points; reviewers read many and value clear, checkable statements.
Where do I submit a Plan of Action?
In Seller Central, through Account Health: open the issue in the notice and use its Appeal button. Do not send it by email.
Can I use the same template for every issue?
The structure stays the same; the content cannot. Amazon rejects Plans of Action that do not address the specific issue and ASINs in the notice.
Can you write it for me?
Yes. Paste the notice for a free diagnosis and evidence check, then get the Plan of Action written from your facts and documents for $79 (account) or $49 (listing), with unlimited revisions.
